Why dermatology is different in urgent care
Dermatologic complaints are common, but the visible lesion is only the beginning of the assessment. The same description—“red, swollen skin”—may represent cellulitis, an abscess, contact dermatitis, a bite reaction, venous stasis, gout, thrombosis, or an early necrotizing infection. A diffuse rash may be a self-limited viral eruption, urticaria, a medication reaction, or the first visible sign of systemic illness.
The urgent-care clinician therefore has three jobs:
- Recognize instability and time-sensitive disease.
- Describe the eruption accurately enough to build a useful differential.
- Choose a treatment and disposition that fit the most likely diagnosis without masking a dangerous alternative.
This article offers a reusable framework. It is not an encyclopedia of every rash and should not encourage diagnostic certainty when the morphology, history, or trajectory remains unclear.
Begin with the emergency decision
Before naming the eruption, assess the patient. Obtain and interpret a complete set of vital signs. Look for toxic appearance, altered mental status, hypotension, respiratory distress, hypoxemia, rapidly progressive disease, and pain that seems disproportionate to the visible findings.
Immediate emergency evaluation is generally appropriate when any of the following is present:
- Airway symptoms, wheezing, hypotension, syncope, or rapidly evolving multisystem symptoms concerning for anaphylaxis
- Severe pain out of proportion, rapidly spreading edema or erythema, bullae, skin anesthesia, crepitus, ecchymosis, necrosis, or systemic toxicity concerning for necrotizing soft-tissue infection
- Widespread skin pain, blistering, epidermal detachment, or mucosal erosions, especially after a new medication
- Nonblanching petechiae or purpura with fever, toxicity, hypotension, severe headache, neck stiffness, or altered mental status
- Facial or periorbital infection with pain on eye movement, ophthalmoplegia, proptosis, vision change, or severe headache
- Extensive burns, electrical or chemical burns, inhalation injury, circumferential burns, or burns involving critical functional areas
- Rapidly progressive infection in a markedly immunocompromised patient
- Concern for sepsis, compartment syndrome, septic arthritis, or deep-space infection
A normal temperature does not exclude serious infection. Older adults, immunocompromised patients, and patients taking antipyretics or corticosteroids may have muted findings.
Describe first, diagnose second
Precise morphology improves reasoning, documentation, consultation, and follow-up. Avoid using “rash” as the entire examination.
Primary and secondary lesions
- Macule: Flat color change, generally less than 1 cm.
- Patch: Larger flat area of color change.
- Papule: Small, solid, raised lesion.
- Plaque: Raised, broad lesion, often formed by coalescing papules.
- Nodule: Deeper, solid lesion extending into the dermis or subcutaneous tissue.
- Vesicle: Small fluid-filled blister.
- Bulla: Larger fluid-filled blister.
- Pustule: Superficial lesion containing purulent material.
- Wheal: Transient, edematous, raised lesion typical of urticaria.
- Petechiae: Pinpoint nonblanching hemorrhagic lesions.
- Purpura: Larger nonblanching hemorrhagic lesions.
- Scale: Flaking stratum corneum.
- Crust: Dried serum, blood, or purulent exudate.
- Erosion: Superficial epidermal loss.
- Ulcer: Deeper loss extending into the dermis or beyond.
- Excoriation: Linear or punctate erosion caused by scratching.
- Lichenification: Thickened skin with accentuated lines from chronic rubbing.
Document color, border, surface, tenderness, warmth, fluctuance, induration, drainage, blanching, and whether the epidermis is intact. “Nonblanching” means pressure does not temporarily remove the color; it is a finding, not a diagnosis.
Distribution and configuration
Distribution often narrows the differential more than lesion color. Note whether the process is localized or generalized; unilateral or symmetric; flexural or extensor; dermatomal; acral; intertriginous; photo-distributed; or limited to palms, soles, scalp, face, or mucosa.
Configuration may be annular, linear, grouped, targetoid, reticular, confluent, discrete, or following a contact pattern. Ask whether lesions remain fixed or migrate. Individual urticarial wheals generally move or resolve within hours; a painful or purpuric lesion fixed beyond approximately 24 hours should broaden the differential.
Skin findings vary across skin tones. Erythema may appear red, violaceous, gray, or dark brown and can be subtle. Palpation for warmth, edema, induration, and tenderness, comparison with unaffected skin, and inspection in good lighting reduce reliance on redness alone.
Focused history
Time and trajectory
Establish when the first lesion appeared, where it began, and how it changed. Rapid expansion over hours is different from a stable eruption evolving over weeks. Determine whether pain, itching, fever, or systemic symptoms preceded the skin findings.
Symptoms
Clarify whether the dominant symptom is itch, pain, burning, numbness, or drainage. Intense itch supports—but does not prove—an allergic, eczematous, infestational, or urticarial process. Prominent pain raises concern for infection, ischemia, zoster, ulceration, or a severe drug eruption.
Exposures and transmission
Ask about new personal-care products, detergents, adhesives, occupational materials, plants, metals, topical medications, hot tubs, animals, insects, travel, sick contacts, shared bedding, sports, shelters, and congregate living. Ask about water exposure, bites, punctures, injection drug use, tattoos, shaving, and recent procedures when infection is possible.
Medication history
Create a timeline of all prescription drugs, over-the-counter products, supplements, and intermittent medications. Include recent antibiotics, anticonvulsants, allopurinol, nonsteroidal anti-inflammatory drugs, and acetaminophen. A medication taken safely in the past can still be associated with a later severe skin reaction. Record the start date, stop date, dose change, and onset of symptoms.
Host factors
Identify diabetes, peripheral vascular disease, chronic edema, eczema, obesity, renal or hepatic disease, malignancy, HIV, neutropenia, immunosuppressive therapy, pregnancy, and prior resistant organisms. Ask about recurrent abscesses, hidradenitis, household infections, previous dermatologic diagnoses, and response to earlier treatment.
Associated features
Review fever, chills, malaise, sore throat, cough, gastrointestinal symptoms, joint pain, headache, neck stiffness, eye pain, dysuria, genital lesions, facial swelling, dyspnea, and mucosal pain. A “skin complaint” with organ symptoms may be systemic disease.
Focused examination
Expose enough skin to determine the true distribution while preserving privacy. Examine the mouth and other mucosa when a medication reaction, autoimmune blistering process, infection, or systemic eruption is possible. Inspect palms, soles, web spaces, nails, and scalp when relevant.
For localized redness or swelling:
- Measure the involved area in centimeters.
- Mark the border with date and time when useful for follow-up.
- Assess fluctuance, drainage, induration, lymphangitic streaking, and regional nodes.
- Evaluate distal perfusion, sensation, and movement when an extremity is involved.
- Examine joints and bursae separately rather than attributing every painful swelling to cellulitis.
- Look for portals of entry such as fissures, ulcers, tinea pedis, wounds, or foreign bodies.
For facial or periorbital disease, document visual acuity when feasible, pupils, extraocular movements, pain with movement, proptosis, and neurologic findings. For a wound, describe depth, contamination, devitalized tissue, foreign-body risk, neurovascular status, and tetanus history.
Clinical photographs can support comparison and consultation, but only with informed consent and secure storage that follows applicable privacy rules. A photograph does not replace a written examination.
A pattern-based differential
Red, warm, tender skin
Nonpurulent cellulitis is a diffuse bacterial infection without a drainable collection. It is generally unilateral and characterized by warmth, tenderness, edema, and spreading erythema. Bilateral lower-leg redness should prompt serious consideration of mimics such as stasis dermatitis, lymphedema, lipodermatosclerosis, contact dermatitis, or systemic edema.
Abscess is a localized collection of pus. Fluctuance, focal tenderness, pointing, or spontaneous drainage supports the diagnosis, but early or deep abscesses may be indurated without obvious fluctuance. Purulent collections require source-control thinking; simply labeling an abscess “cellulitis” can lead to antibiotic-only treatment when drainage is the primary intervention.
Inflammatory mimics include contact dermatitis, arthropod reactions, gout, pseudogout, superficial thrombophlebitis, deep venous thrombosis, venous stasis, and ruptured cysts. Sharp geometric borders, intense itch, vesiculation, bilateral involvement, chronic pigment change, or lack of tenderness may point away from uncomplicated cellulitis.
Necrotizing soft-tissue infection can initially resemble cellulitis. Severe pain, pain outside the visible erythema, rapid progression, systemic findings disproportionate to the skin examination, tense edema, bullae, ecchymosis, anesthesia, crepitus, or necrosis are major warnings. Imaging or a reassuring laboratory score must not delay surgical evaluation when suspicion is high. CDC guidance identifies early aggressive surgical exploration and debridement, with parenteral antibiotics, as the core treatment.[1]
Pruritic eruption
Contact dermatitis may be irritant or allergic. Distribution is often the clue: linear plant exposure, a watchband pattern, adhesive outline, glove distribution, or rash confined to a newly treated area. Vesicles and weeping may occur acutely; scale and lichenification suggest chronic exposure. Management depends on removal of the trigger, skin-barrier care, and appropriately selected topical anti-inflammatory therapy.
Urticaria consists of transient wheals caused by dermal edema. Ask whether each lesion migrates or disappears within a day. Evaluate for airway, breathing, circulatory, and gastrointestinal involvement. Urticaria alone is not anaphylaxis; conversely, anaphylaxis can occur without visible hives. Intramuscular epinephrine is the first-line treatment for anaphylaxis. Antihistamines are adjuncts and must not delay epinephrine or emergency care.
Scabies should be considered with nocturnal itch, affected close contacts, and lesions in finger webs, wrists, waistline, axillae, genital region, or other characteristic sites. Infants, older adults, and immunocompromised patients may have atypical or extensive involvement. Treatment planning includes appropriate patient therapy, simultaneous management of close contacts when indicated, and laundering or isolation of recently used clothing and bedding.
Tinea often presents as an annular, scaly plaque with an active border, but morphology can be distorted by corticosteroids. A steroid may reduce redness while allowing fungal infection to extend—sometimes termed tinea incognito. Avoid reflexively prescribing combination antifungal-corticosteroid products when the diagnosis is uncertain.
Vesicles, bullae, erosions, or skin pain
Grouped unilateral vesicles in a dermatomal distribution suggest herpes zoster. Evaluate the eye and tip or side of the nose when the ophthalmic division is involved, and urgently escalate ocular symptoms or findings. Disseminated lesions, major immunocompromise, neurologic symptoms, pregnancy-related concerns, or inability to maintain oral therapy may change disposition.
Diffuse skin pain, targetoid or dusky lesions, blistering, epidermal detachment, and mucosal erosions raise concern for Stevens–Johnson syndrome or toxic epidermal necrolysis. Stop suspected nonessential culprit medication when clinically appropriate, do not rechallenge it, and arrange emergency hospital evaluation. The FDA warns that serious reactions including SJS, TEN, and acute generalized exanthematous pustulosis can occur with acetaminophen and can occur even after previous uneventful use.[2]
Drug reaction with eosinophilia and systemic symptoms (DRESS) may begin weeks after a new medication and can involve fever, facial edema, lymphadenopathy, hematologic abnormalities, and liver, kidney, lung, heart, or pancreatic injury. The skin appearance alone does not determine severity. The FDA notes that DRESS commonly begins two to eight weeks after initiating the implicated medicine.[3]
Bullae can also arise from burns, edema, friction, bullous impetigo, autoimmune disease, diabetic bullae, and vascular compromise. Do not intentionally rupture an unexplained tense bulla merely to “see what is underneath.”
Petechiae and purpura
First confirm nonblanching morphology. Then assess the entire patient. Fever or toxicity with rapidly evolving purpura demands emergency evaluation for invasive infection, sepsis, disseminated intravascular coagulation, or purpura fulminans. Other possibilities include thrombocytopenia, medication effects, small-vessel vasculitis, trauma, dependent pressure, and benign localized petechiae.
Distribution, platelet-related bleeding symptoms, abdominal or joint symptoms, renal symptoms, medication history, and hemodynamic status guide testing and disposition. “Petechiae” should not be used as a reassuring final diagnosis.
Ulcers and wounds
Determine whether the wound is traumatic, neuropathic, arterial, venous, pressure-related, inflammatory, infectious, or malignant. Measure length, width, and depth; document the wound base, drainage, odor, surrounding tissue, undermining, exposed structures, perfusion, sensation, and probe-to-bone findings when appropriate.
Poor perfusion, deep infection, exposed tendon or bone, rapidly progressive necrosis, systemic illness, uncontrolled diabetes, or inability to perform wound care should lower the threshold for escalation. Antibiotics do not substitute for debridement, drainage, pressure relief, vascular assessment, glycemic management, or foreign-body removal when those are required.
Testing: use it to answer a question
Many eruptions are clinical diagnoses. Testing is most useful when it can meaningfully change treatment or disposition.
- Point-of-care ultrasound can help distinguish cellulitic edema from a drainable collection and can identify deeper or multiloculated disease. Operator skill and anatomic limitations matter.
- Culture purulent material when disease is severe, recurrent, unusual, treatment has failed, or host factors make the result important. Routine swabbing or aspiration of intact uncomplicated cellulitis is not recommended.[4]
- KOH preparation, when available and performed competently, can support a dermatophyte or yeast diagnosis.
- Viral testing may be appropriate for atypical, disseminated, occupationally consequential, or high-risk vesicular disease.
- Blood tests may be warranted for systemic illness, severe medication reaction, vasculitis, hematologic disease, organ involvement, or complications, but are not routinely necessary for a stable, localized uncomplicated eruption.
- Radiography may identify radiopaque foreign material, bony involvement, or occasionally gas. A negative radiograph does not exclude radiolucent foreign material or necrotizing infection.
Avoid ordering tests to create false reassurance. If the patient requires emergency consultation based on the clinical picture, transfer should not be delayed for nonessential outpatient testing.
Treatment principles and safeguards
Cellulitis
For typical mild nonpurulent cellulitis without systemic signs, therapy should target streptococci. Broader methicillin-resistant Staphylococcus aureus coverage is considered when there is purulence or relevant risk such as penetrating trauma, MRSA elsewhere or colonization, injection drug use, or severe systemic illness. IDSA recommends outpatient treatment only when the patient lacks systemic inflammatory response, altered mental status, or hemodynamic instability; concern for deep infection, severe immunocompromise, adherence barriers, or treatment failure favors hospitalization.[4]
Select the agent using current local guidance. Check allergy phenotype, pregnancy status, renal and hepatic function, interactions, and recent antibiotics. Elevation and treatment of portals of entry—such as interdigital fissuring or tinea pedis—are part of care. Explain that visible redness may not disappear immediately; worsening systemic symptoms, rapid spread, new severe pain, or failure to improve requires reassessment.
Abscess
Incision and drainage is the principal treatment for an accessible uncomplicated cutaneous abscess. Antibiotics are an adjunct based on systemic findings, surrounding infection, lesion burden or location, recurrence, immune status, age extremes, failed initial therapy, and local guidance.[4]
Before performing drainage, consider proximity to major vessels, nerves, joints, the eye, breast, hand, perineum, or cosmetic structures; depth; suspected foreign body; hidradenitis; pilonidal disease; and whether sedation, imaging, or specialty care is required. Document consent, anesthesia, incision, exploration, irrigation if used, packing decision, specimen collection, blood loss, tolerance, and follow-up.
Topical corticosteroids
Match potency and vehicle to the diagnosis, body site, severity, age, and planned duration. Thin skin of the face, eyelids, folds, and genital region is more vulnerable to atrophy and other adverse effects. Occlusion increases absorption. Use extra caution in children and over large surface areas.
Do not use a topical steroid as a diagnostic trial when untreated fungal, bacterial, viral, or parasitic infection remains plausible. Systemic or depot corticosteroids can temporarily suppress visible inflammation and fever, worsen glycemia, and complicate infection recognition. A nonspecific “rash shot” is not a substitute for diagnosis, safety-netting, and follow-up.
Antihistamines and anaphylaxis
For uncomplicated urticaria without anaphylaxis, a second-generation H1 antihistamine is often preferred because it is generally less sedating than first-generation agents. Counsel about medication-specific sedation and impairment. For suspected anaphylaxis, use intramuscular epinephrine promptly, activate emergency response, monitor, and transfer according to protocol. Corticosteroids and antihistamines do not reverse airway obstruction or shock.
Antimicrobial stewardship
Not every crust, wound, or red plaque needs an antibiotic. Conversely, a prescribed antibiotic does not cure a drainable abscess or necrotic tissue. State the working diagnosis, suspected pathogen class, treatment goal, expected response, and reassessment threshold. Avoid adding MRSA coverage to uncomplicated nonpurulent cellulitis without a clinical reason.[4]
Wound and burn care
Clean wounds, remove visible contaminants when safe, assess for retained foreign body and devitalized tissue, and choose a dressing that protects the wound while managing moisture. Review tetanus vaccination and tetanus immune globulin indications based on wound type and immunization history; antibiotics are not used solely to prevent tetanus.[5]
Burn assessment includes mechanism, time, depth, total body surface area, location, circumferential involvement, neurovascular status, inhalation risk, associated trauma, and host factors. Follow current burn-center referral guidance for potentially deep burns, large burns, critical areas, inhalation injury, chemical or electrical injury, poorly controlled pain, significant comorbidity, and pediatric needs beyond local capability.[6]
Disposition and follow-up
Discharge is safest when the patient is clinically stable, the diagnosis and dangerous alternatives have been considered, the treatment can be carried out, and follow-up is realistic.
Give diagnosis-specific instructions plus explicit emergency precautions. Depending on the condition, return immediately for:
- Trouble breathing, throat tightness, faintness, or facial/tongue swelling
- Fever with worsening illness or confusion
- Rapid spread beyond a marked border
- New severe or disproportionate pain
- Dusky skin, blistering, numbness, black discoloration, or crepitus
- Eye pain, vision change, painful eye movement, or new facial swelling
- Mouth, eye, or genital erosions
- Nonblanching rash with systemic symptoms
- Inability to take medication, maintain hydration, or perform wound care
A planned recheck in approximately 24–48 hours is often appropriate for cellulitis, a drained abscess, an uncertain eruption, high-risk host factors, or disease near a critical structure. Earlier review is warranted if the trajectory changes. When dermatology access is limited, do not allow a routine referral to replace emergency escalation for a time-sensitive condition.
Documentation that supports safe care
A strong note makes the clinician’s reasoning visible. Include:
- Onset, progression, symptoms, exposures, medication timeline, prior treatment, and host risks
- Relevant vital signs and general appearance
- Morphology, color, size, distribution, configuration, blanching, warmth, tenderness, induration, fluctuance, drainage, and mucosal involvement
- Relevant ocular, neurologic, vascular, joint, and systemic findings
- Important dangerous alternatives considered and the findings for or against them
- Tests performed, their limitations, and how results changed management
- Procedure details and consent when applicable
- Medication decision, allergy assessment, and relevant dosing considerations
- Disposition rationale, follow-up interval, and specific return precautions
- Consultation or transfer details and informed refusal if applicable
Example medical-decision-making language
Patient is clinically stable and nontoxic with a localized unilateral area of warmth, tenderness, and poorly demarcated erythema. No fluctuance, drainage, bullae, crepitus, anesthesia, pain out of proportion, rapid progression, hypotension, altered mental status, or joint-centered pain. Findings currently favor uncomplicated nonpurulent cellulitis; abscess, necrotizing infection, septic joint, and inflammatory mimics were considered. Outpatient therapy is reasonable given stable vital signs, oral tolerance, reliable follow-up, and absence of severe immune compromise. Border marked. Recheck arranged within 24–48 hours, with immediate emergency precautions for rapid spread, severe pain, systemic illness, blistering, discoloration, numbness, or clinical deterioration.
Adapt this language to the actual encounter. Never import normal findings that were not assessed.
Informed refusal
If a patient declines emergency transfer, a recommended procedure, medication, or further evaluation, assess decision-making capacity and explain the suspected condition, material risks of refusal, alternatives, and warning symptoms. Offer the safest feasible alternative without representing it as equivalent. Document the patient’s stated reason, questions, understanding, decision, witnesses when relevant, and follow-up plan. Provide emergency precautions in writing.
Coding considerations
Code the diagnosis supported at the end of the encounter, not the most dramatic item in the differential. Examples that may apply include:
- R21 — Rash and other nonspecific skin eruption
- L03.90 — Cellulitis, unspecified
- L02.91 — Cutaneous abscess, unspecified
- L50.9 — Urticaria, unspecified
- L25.9 — Unspecified contact dermatitis, unspecified cause
- B35.9 — Dermatophytosis, unspecified
- B02.9 — Zoster without complications
More specific site, laterality, cause, complication, encounter phase, and burn depth or body-surface codes may be required. Verify the current ICD-10-CM code set and payer rules.
Common procedure families include simple or complicated incision and drainage of a cutaneous abscess and removal of a subcutaneous foreign body. Code selection depends on what was actually performed and documented; evaluation-and-management services, supplies, ultrasound, repair, or burn care have separate rules. Do not select a code from a short educational list without checking the current descriptor, bundling edits, documentation, and payer policy.
Clinical pearls
- Describe the lesion before naming it.
- Bilateral lower-leg erythema is often a cellulitis mimic.
- Purulence calls for source-control thinking.
- Pain out of proportion is a warning, not a diagnosis; act on the whole trajectory.
- Do not delay epinephrine in anaphylaxis while waiting for hives.
- Mucosal erosions plus skin pain or blistering require urgent escalation.
- A new medication timeline may be more important than the rash photograph.
- Steroids can distort infection and tinea.
- Marking and measuring a lesion makes follow-up more objective.
- Skin tone changes how erythema appears; palpation and comparison matter.
- A negative test does not overrule a dangerous clinical pattern.
- When certainty is limited, state it and build a safer follow-up plan.
Conclusion
Urgent-care dermatology is less about memorizing hundreds of named eruptions than consistently answering a sequence of practical questions: Is the patient unstable? Is this skin-limited or systemic? What is the morphology and distribution? Is there purulence, mucosal disease, vascular compromise, severe pain, or rapid progression? Does the proposed treatment address the actual process, and could it conceal a dangerous alternative?
A morphology-first examination, disciplined search for red flags, thoughtful antimicrobial and corticosteroid use, and explicit reassessment plan make dermatologic care safer. When the findings suggest anaphylaxis, necrotizing infection, severe cutaneous drug reaction, invasive infection, ocular involvement, or another threat to life or function, the correct urgent-care intervention is timely stabilization and escalation—not a more elaborate outpatient label.
